*Job Description*
My Choice Therapy is looking for a Patient Care Coordinator to enhance the quality of patient management and maximize patient satisfaction.
_*Duties*_
* The incumbent will be accountable for the full patient care cycle.
* Assess, plan, implement, monitor and evaluating actions required to meet the client’s home health services needed.
* Coordinate care that is safe, timely, effective, efficient, equitable, and client-centered
* Handle case assignments, revise care plans, review case progress and manage case closure
* Develop effective working relations and cooperate with medical team throughout the entire case management process
* Take the extra mile and interact with patients to keep track of their progress and to ensure satisfaction
* Record cases information, complete accurately all necessary forms and produce statistical reports
* Adhere to professional standards as outlined by protocols, rules and regulations
_*Qualifications*_
* Effective communication skills
* Excellent organizational and time management skills
* Familiarity with EMR systems
* Problem solving skills and ability to multi-task
* Compassionate with teamwork skills
