We are seeking a full-time, bilingual, and remote Licensed Vocational Nurse (LVN/LPN) to join our rapidly growing team as a Care Coordinator.
You will be responsible for telephonically delivering high quality care, coaching, and appointment scheduling to assigned patients.
Responsibilities:
• Remotely provide patient health coaching and care to improve patient outcomes
• Coach and educate patients on improving their Chronic Conditions, preventive care and physician directives
• Update and manage patient care plans based on assessment of patients’ needs and physician directives
• Uphold scheduling flexibility between the hours of 8:00am – 7:00pm (PST) to call patients who live in different time zones
Requirements:
• Valid Vocational Nurse (LVN/LPN) License
• Valid Vocational Nurse (LVN/LPN) License - in the state of California is mandatory
• 2 years+ Licensed Vocational Nurse (LVN/LPN) experience
• Bilingual (Fluent in English and Chinese (Cantonese/Mandarin))
• Experience working with EHRs/EMRs
• Strong knowledge and background of chronic conditions
• Ability to build rapport with patients
• Compassionate and caring demeanor
• Excellent documentation and verbal communication skills
• Intermediate to advanced computer skills with the ability to navigate multiple systems
• Reliable internet connection
