*JOB DESCRIPTION*
Buena Vista Hospice is seeking an Admission Registered Nurse (RN) to join our team and be the first face a new patient sees.
The Hospice Admission RN performs consultation and admission visits to assess the patient’s needs and completes admission assessment.
The schedule for this position is Tuesday – Saturday and will serve Ventura County and the San Fernando Valley.

*Regular Schedule: 1:30 pm - 9:00 pm*
* Complete an initial and comprehensive assessments of patient and family to determine hospice need
* Provide a complete physical assessment and history of current and previous illness(es)
* Provide professional nursing care by utilizing all elements of nursing process
* Assess and evaluate patient’s status
* Use health assessment data to determine nursing diagnosis
* Develop a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions that includes patient and family in the planning process
* Initiate appropriate preventive and rehabilitative nursing procedures
* Administer medications and treatments as prescribed by the physician in the physician’s plan of care
* Counsel the patient and family in meeting nursing and related needs
* Provide health care instructions to the patient as appropriate per assessment and plan
* Assist the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care
* Communicate with clinical team of patient’s care in the future
*JOB REQUIREMENTS*
* Registered Nursing License
* 1+ Year of Hospice Experience
* Ability to perform prolonged or considerable walking or standing
* Ability to lift, position, and/or transfer patients
* Ability to lift supplies and equipment
* Ability to perform considerable reaching, stooping, bending, kneeling, and/or crouching
* Possess visual acuity and hearing to perform required nursing skills
* Maintain a current CPR certification
