We are searching for a professional & compassionate Full-time Licensed Vocational Nurse (LVN) to join us in this mission.
*Job Description*
* Demonstrates commitment, professional growth and competency.
* Implement plan of care initiated by the Case Manager or Registered Nurse.
* Provide accurate nursing notes/documentation in an accurate, legible and timely manner consistent with the plan of care.
* Assess and provide patient and family education related to diagnosis and plan of care.
* Reporting changes in the patient’s conditions and needs to the Case Manager or Clinical Manager.
* Demonstrates flexibility in patient scheduling and readily accepts new assignments.
*Requirements:*
* Current LVN license in CA
* Valid driver's license, auto insurance and dependable vehicle
* Minimum 1 year experience as LVN
* Home Health/Med Surg experience preferred
* Strong assessment skills
