*Per-Diem Rate of Pay:* $260 per admission, $100 per routine visit, $200 per recert

The Registered Nurse plans, organizes, and directs hospice care and is experienced in nursing, with an emphasis on community health education/experience.
The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
*ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES*
*Patient Care*
Completes an initial assessments of patient and family to determine hospice needs.
Provides a complete physical assessment and history of current and previous illness(es).
Provides professional nursing care by utilizing all elements of nursing process.
Assesses and evaluates patient’s status by:
* Writing and initiating plan of care
* Regularly evaluating patient and family/caregiver needs
* Uses health assessment data to determine nursing diagnosis.
* Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.
Includes the patient and the family in the planning process.
* Provides health care instructions to the patient as appropriate per assessment and plan.
*Additional Duties*
* Participates in on-call duties as defined by the on-call policy.
* Ensures that arrangements for equipment and other necessary items and services are available.
* Supervises ancillary personnel and delegates responsibilities when required.
*Shift Requirements:*
* Will be discussed during interview.
Rotation of weekends are required.
*Required Qualifications:*
* At least 1 year experience in Hospice within that last 3 years
* Current California RN license
* Excellent written and verbal communication skills
* Valid CA Driver’s License and auto insurance
* Current CPR
* Current TB/Physical
* Pass Background Clearance
*Preferred Qualifications:*
* Bilingual (English/Spanish)
* Baccalaureate degree in nursing
