Northridge Care Center is looking for a Part-time MDS Coordinator to join our team! The MDS Coordinator completes MDS for all residents according to OBRA and PPS guidelines.
Assists in providing a clean, safe, happy and healthy environment for residents by performing the duties as described below.
The goal is to promote the patient’s well-being by providing high quality nursing care.
MDS Coordinator Requirements
Valid and current California RN License
1-2 years MDS experience or MDS completed course
BLS or CPR (required)
1-2 years experience in MDS coordinator role or related experience required
Exceptional mathematical skills and ability to apply them to tasks such as analysis of variance and sampling theory
Deep medical terminology knowledge
Experience in a clinical and healthcare setting preferred
Excellent leadership skills
Demonstrable knowledge of state and federal regulations
Superb decision-making skills
Previous experience in a SNF or similar job setting would be preferred
Excellent knowledge of nursing care methods and procedures.
Excellent knowledge of emergency care.
In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.
) and willingness to follow them at all times
A team player with excellent communication and interpersonal skills
Primary MDS Coordinator Responsibilities
Participate in the admission process of prospective residents in terms of their nursing needs and appropriate placement
Determine potential Resource Utilization Groups (RUGs) and expense associated with a potential admission
Review all applications for admission, followed by on-site or phone assessment and communicate results of assessment to the Admissions Coordinator
Complete and assure the accuracy of the MDS process for all residents
Maintain current working knowledge of Medicare criteria, serving as a resource for nursing staff and communicate changes in regulations
Monitor Case Mix Index (CMI) scores, looking for potential risks and/or changes that may affect Medicaid reimbursement
Facilitate problem-solving for complicated admissions
Monitor Medicare assessment schedules and nursing documentation to ensure accuracy and timely submission
Complies with the facility’s attendance policy requiring regular, reliable, and punctual attendance
Other Responsibilities
Adjust and administer patient’s medication and provide treatments according to physician’s orders
Ensures timely response to call lights
Pain management/intervention
Assess behavior and provide interventions as indicated
Wound treatments
Therapy treatments
Incidents; falls, etc.
Patient complaints / concerns
Identifies/assesses emergency medical situations and provides appropriate interventions as indicated
Documentation and follow-up with timely notification of physician and family
Care planning as needed
Performs other duties or functions as assigned by the DON and/or Administrator
PAY SCALE: $45 - $50 / Hour
Depending on a number of factors, including, but not limited to location, regularly scheduled work shift, knowledge, skills, experience, and expertise.
