Case Manager/Intake coordinator will be responsible for gathering medical histories, and obtaining any health insurance information family has.
As an case manager/intake coordinator, you should be highly organized, detail-oriented, have excellent interpersonal skills with a strong aptitude for great customer service.
QUALIFICATIONS:
One (1) year Hospice experience, MUST.
LVN preferred but not required.
Excellent organizational and time-management skills.
Ability to supervise, guide and develop skills and performance of service personnel.
Ability to exercise initiative and independent judgment.
Ability to work in a fast-paced environment.
Great communication and interpersonal skills.
RESPONSIBILITIES:
Coordinates patient care and referrals.
Assures physician approval of plans for continuity of care.
Observes confidentiality and safeguards all patient related information.
Serves as a resource person to employees.
Develops a cooperative relationship and communicates effectively and professionally with the physicians.
Reports any problem relating to patient care or conditions.
Assures that patients’ plans of care are developed, implemented and updated.
Ensures that all necessary supplies and equipment are available.
Is normally available at all times during and after operating hours; may designate a qualified temporary replacement if he/she will not be available.
Makes patient and personnel assignments.
Addresses and corrects all patient concerns.
Print, collect, organize, and manage all accepted referrals, the process for information verification, eligibility check, and data entry.
Answering telephone calls with the first 3 rings and transferring to the appropriate person.
Constantly checking email and fax for updates.
Verify patient insurance coverage and benefits.
Staffing and plotting Nurses within 24 hours
Updating patient census list daily
Inputting patient information on HospiceMD.
Uploading patient’s documents (Referral/H&P/Face Sheet/CC notes/CTIs) on HospiceMD
Preparing admission packets and office charts
Arranging transportation for patients
Requesting and inputting Continuous Care notes on HospiceMD
Sending authorization to insurance companies
Maintain a professional, positive, and motivated working environment.
Maintain good communication and professional relationships with the staffing department.
Understands and adheres to established policies and procedures.
