Primary Purpose and Function Under the direction of the Director of Aftercare & Alumni Services, the Aftercare & Alumni Coordinator/ Counselor is responsible for assisting with case-management services related to aftercare planning for clients at our outpatient and residential centers, for conducting scheduled follow-up calls and emails to alumni of our residential and outpatient centers, and for aiding with tasks related to the agency’s alumni services program.
Essential Functions and Responsibilities 1.
Grow and maintain Alumni membership.
2.
Maintain Facebook and other Social Media Groups.
3.
Assist Case Management and Clinical Director in educating new patients in residential and detox services regarding AA, NA, SMART Recovery, Refuge Recovery, and other community-based support services.
4.
Assist Tech Supervisor, Case Manager, and Clinical Director in tracking and preventing untimely launching of MMPI, Basis 24 initial and midpoint measures for residential clients.
5.
Manage Constant Contact system to electronically manage communications with all current and alumni clients (bulk emails).
6.
Accurately input Alumni data in KIPU’s CRM that result in outcome data and studies.
7.
Facilitate COVID approved Experiential Outings.
8.
Assist with collection of consents for treatment and welcome of new admissions to residential and detox services.
9.
Assists the Case Manager and Clinical Director with clients needing assistance with aftercare planning on a weekly or biweekly basis to address specific case-management needs, including but not limited to needs for employment, housing, and continued treatment, until case-management needs are resolved.
10.
Document all meetings with clients in agency’s Electronic Records System.
11.
Assists in the production of graduation and discharge material of patients in detox and residential.
12.
Assists Case Manager and Clinical Director with regular to weekly electronic updates on the aftercare plans of all clients in residential and detox services.
13.
Maintain professional appearance and healthy boundaries with colleagues and clients.
14.
Assist with tasks related to running of the Alumni Services Program, including but not limited to ordering food, purchasing group supplies, creating flyers and announcements, and obtaining required consent forms.
15.
Provide coverage of alumni groups and events as needed and document attendance of all current clients in agency’s Electronic Record System.
16.
Follow all agency policies and procedures related to aftercare and alumni services.
17.
Attend required individual and/or group supervision sessions.
18.
Assist with the collection of and tracking of client satisfaction and outcome surveys.
19.
Conduct scheduled follow up calls to alumni of our outpatient and residential programs and document accurately in our tracking system.
20.
Follow all procedures for tracking client outcomes.
21.
Assist with the distribution of alumni and patient surveys.
22.
Perform related duties and participate in special projects as assigned.
23.
Discuss Client Handbook during orientation, AND coordinate benefits by calling the insurance company with the patient and verifying they only have one insurance with the insurance company.
24.
Facilitate two evening Alumni Weekly Meetings.
Requirements: Minimum Qualifications · Registered MSW Associate, ACSW/AMFT Intern, Certified Alcohol Drug Counselor I or II, eligible within 30 days of hire.
· Experience relevant to working in addiction treatment.
· Excellent problem-solving skills.
· Must be able to write clearly and informatively.
· Computer literacy and knowledge of Windows, Word, Excel, and familiarity with Electronic Record Systems.
· Maintains appropriate registration with the State of California's Board of Behavioral Sciences (BBS) as an AMFTI or with California Consortium of Addiction Programs and Professionals (CCAPP) as a CADC-I or CADC-II, or RADT or it is equivalent including completion of state required continuing education credits, hours of supervision, and other procedures required by the relevant board.
· Reliable transportation, valid California Driver’s License with a clear driving record and proof of insurance.
· DHCS Standard 13010(b)(6) requires personnel file to contain health screening report and tuberculosis test results.
· Current CPR/ first aid training.
· 2 years sobriety
