```Duties``` - Provide direct patient care in various settings, including acute care and primary care - Perform assessments, monitor vital signs, and administer medications as prescribed - Assist with activities of daily living, such as bathing, dressing, and feeding - Administer tube feedings and monitor patients' nutritional status - Collaborate with healthcare team members to develop and implement individualized care plans - Document patient information accurately and timely using electronic medical records (eClinicalWorks) - Educate patients and their families on disease management, medication administration, and self-care techniques - Coordinate discharge planning and ensure a smooth transition of care ```Requirements``` - Valid nursing license in the state of [Location] - Minimum [X] years of experience in a healthcare setting - Proficient in medical terminology and medical office procedures - Basic Life Support (BLS) certification - Strong knowledge of vital signs monitoring and interpretation - Experience with tube feeding administration and management - Familiarity with electronic medical record systems (eClinicalWorks preferred) - Ability to work in both acute care and primary care settings - Excellent communication and interpersonal skills - Strong organizational skills and attention to detail Note: This job description is not intended to be all-inclusive.
