*CASE MANAGER / HOUSING NAVIGATOR with One Stop Emergency Services Program* *JOB DESCRIPTION* *Responsibilities and Duties:* *Responsibilities and Duties:* Pukúu Cultural Community Services (Pukúu CCS) seeks a Case Manager/Housing Navigator to join our One Stop Emergency Services (OSES) team to help Native Americans in accessing resources including housing and supportive services.
Through OSES, Pukúu CCS focuses on providing one-on-one support to families and individuals facing deep poverty and multiple special need to help them achieve stable and lasting wellness.
As a case manager, you will assist families and individuals facing multiple needs, specifically those at risk of or experiencing homelessness to link them to the necessary resources like safe and affordable housing and other housing resources, food and transportation assistance, and other community resources, geared towards stability and wellness.
You will work closely with landlords, authority personnel, and organizations that provide social services.
*About the Program:* *One Stop Emergency Services (OSES) *is a program of Pukúu Cultural Community Services, a Native American serving non-profit founded by the local Tribe, the Fernandeño Tataviam Band of Mission Indians.
OSES was created to tackle economic disparities faced by Native Americans in Los Angeles County while providing and connecting community members to social service supports while promoting culturally based activities that protect and promote health and well-being.
This includes keeping AIAN individuals/ families in their homes, making sure they have adequate utilities and ensuring that folks can procure emergency items and needs.
Currently, OSES provides rental assistance, rent burden driven subsidy program, and hotel/motel voucher stays for AIAN and communities in need of stable housing.
*Primary Job Duties:* The Case Manager may expect the following: 1.
Creates individualized services/needs plans in collaboration with families and individuals to address barriers and meet their desired goals that may include housing stability, utility assistance, food security and link to other supportive services.

Provide referrals and direct linkages, such as but not limited to, additional local supportive services, resources, and groups to increase wellness and self-sufficiency.
3.
Work collaboratively with other agencies, such as LAHSA, the Los Angeles City/County Native American Indian Commission, housing providers, and social service providers to support individuals/families progress towards stability and stable housing.
4.
Serve as a liaison to landlords and conduct landlord engagement activities focused on building relationships with landlords and property owners to explain third party payment programs to encourage landlord participation.
5.
Outreaching to services providers, housing entities, and community-based organizations to identify new and existing housing opportunities, supportive services, and mainstream benefits.

* Participate in daily morning manager check-ins and attend all staff meetings.
* Organizes, manages, and maintains accurate online folder tree filing.
* Categorizes, organizes, and labels all incoming paperwork including client notes daily via the Homeless Management Information System (HMIS), Salesforce, and SharePoint within 48 hours.
* Assists with new file clean-up, sets up and manages projects, as needed.
* Responds to calls/emails promptly, schedules appointments with current and prospective clients.
* Routine office tasks such as printing/copying/scanning/filing/mailing * Provide individual case management referrals, and basic need linkages not limited to additional supportive services for individuals/families.
* Process applications, screen referrals, provide intakes and conduct ongoing assessments of clients and prospective clients.
* Provide crisis management services to support individuals and families from various experiences and backgrounds in a way that upholds their dignity and self-determination.
* Provide ongoing outreach, marketing, and client/community cultivation.
* Work occasional evenings and weekends as needed * Responsible for reporting abuse, as a Mandated Reporter which includes immediately reporting any concerns with respect to violation of a person's rights, actual and/or suspected abuse.
* Present client case reviews to the Administration team on a weekly basis.
*Qualifications:* * Bachelor’s Degree in a field pertaining to Social Services * Must provide 3 professional references.
* Two (2) years case management/client advocacy services with families or individuals experiencing homelessness or at-risk of homelessness.
* Six (6) months experience utilizing evidence-based practices in a social services field * Ability to multi-task and have excellent management, planning, and organizational skills * Proficient in general computer use (this will be tested).
* Must have intermediate skills in technological applications and online platforms such as Zoom, and Microsoft Word, Excel, PowerPoint, Outlook, SharePoint, and Teams.
* Must be able to travel and participate in local, regional, and state training as recommended to meet the goals and objectives of the project.
* Ability to meet California minimum and Pukúu insurance requirements * Complete a Background check (security and trust are of the upmost importance).
* Required work authorization (verification of the identity and employment authorization of individual applying).
* Valid California driver's license and good driving record.
* Access to a personal vehicle to be used to conduct Pukúu business *Required Experience:* * One (1) year of progressive experience in administrative or program related work.
